In Frontpage it was just insert then form, but with MS Sharepoint Designer it is different.
How do I do a contact form in Sharepoint designer?
One of those forms that you can put your CGI scripts so it would e-mail youthe form.

Insert > Sharepoint Controls >
or use the Toolbox when you’re on a page
